Japan centrally controls their health care system, including hospital triage standards. From the start of the pandemic, Japan ordered their hospitals to refuse admission to anyone who had not been through 4 days with a 99.5℉ fever, with exceptions only for the elderly, or those with pre-existing conditions.

This common-sense requirement kept their hospitals from being overwhelmed, and preserved a sterile environment in which their existing patients could safely receive treatment for cancer, heart disease, etc.

Japan had a normal cold season for three years with Covid-19, and did not see the extreme levels of deaths of those who would have been seeking hospital treatment for pre-existing conditions, as seen in countries with uncontrolled hospital admissions standards, such as the United States.

Their Practical Maximum Deaths-Per-Case was 2.3%, and is currently at 0.2%.
